PSA: Please read this before posting

Hi,

Welcome to r/pathology. Pathology, as a discipline, can be broadly defined as the study of disease. As such it encompasses different realms, including biochemical pathology, hematology, genetic pathology, anatomical pathology, forensic pathology, molecular pathology, and cytopathology.

I understand that as someone who stumbles upon this subreddit, it may not be immediately clear what is an "appropriate" post and what is not. As a general rule, this is for discussion of pathology topics at a postgraduate level; imagine talking to a room full of pathologists, pathology residents and pathology assistants.

Topics which may be of relevance to the above include:

  • Interesting cases with a teaching point
  • Laboratory technical topics (e.g. reagent or protocol choice)
  • Links to good books or websites
  • Advice for/from pathology residents
  • Career advice (e.g. location, pay)
  • Light hearted entertainment (e.g. memes)
  • "Why do you like pathology?"
  • "How do I become a pathologist?"

Of note, the last two questions pop up in varying forms often, and the reason I have not made a master thread for them or banned them is these are topics in evolution; the answers change with time. People are passionate about pathology in different ways, and the different perspectives are important. Similarly, how one decides on becoming a pathologist is unique to each person, be it motivated by the science, past experiences, lifestyle, and so on. Note that geographic location also heavily influences these answers.

However, this subreddit is not for the following, and I will explain each in detail:

  • Interpretation of patient results

This includes your own, or from someone you know. As a patient or relative, I understand some pathology results are nearly incomprehensible and Googling the keywords only generates more anxiety. Phrases such as "atypical" and "uncertain significance" do not help matters. However, interpretation of pathology results requires assessment of the whole patient, and this is best done by the treating physician. Offering to provide additional clinical data is not a solution, and neither is trying to sneak this in as an "interesting case".

  • University/medical school-level pathology questions

This includes information that can be found in Robbins or what has been assigned as homework/self study. The journey to find the answer is just as important as the answer, and asking people in an internet forum is not a great way. If there is genuine confusion about a topic, please describe how you have gone about finding the answer first. That way people are much more likely to help you.

  • Pathology residency application questions (for the US)

This has been addressed in the other stickied topic near the top.

Posts violating the above will be removed without warning.

Thank you for reading,

Dr_Jerkoff (I really wish I had not picked this as my username...)

Sad to see Holmes duping people again. As a pathologist, I reply to one of her less competent defenders here https://esmoriarty.substack.com/p/that-was-the-tweet-that-was-24-october

Hi everyone!

We're a team of biomedical engineering students at UC Davis working with researchers and clinicians on improving intraoperative classification of ovarian masses (benign, borderline, and malignant).

We're conducting a short 5–10 minute survey to better understand current clinical workflows, challenges with frozen-section diagnosis, and unmet needs.

We're primarily looking for gynecologists, gynecologic oncologists, surgeons, and pathologists with relevant experience.

We'd really appreciate your input! If you're eligible, please consider completing our survey or sharing it with colleagues.

Regret(kind of)

Working as an attending in a South Asian country for the last year or so. The lifestyle is good(not as great as was imagined pre-residency though), but sometimes I wish I'd gone into a clinical branch which would've given me more freedom to earn more money.

The market for pathology here is really saturated, and starting your own set-up is a daunting task. I sometimes dream of how I could've set up my own practice with just an opd and a few basic necessities if I'd gone into clinicals. But hey what's done is done I guess.

Microinvsive focus in a papillary intracholecystic neoplasm?

Thumbnail gallery

Curious about your opinion on this case. Clinical suspicion of gallbladder cancer. Grossing showed a large partly calcified gallbladder with a papillary tumor measuring about 2 cm in diameter, next to a polypoid necrotic mass. I submitted about 80% of the gallbladder wall (26 blocks). In only one block I found this tiny focus. The necrotic focus had no viable tissue.

I am tempted to ignore it as a tangetial cut, especially since it doesn't go deeper than the surface epithelium on the top of the image. Would you call it microinvasion?

Starting MD Pathology residency after 4 yr gap. Need advice!

Hi everyone!

I’ll be starting my MD Pathology residency soon, and I could really use some advice and guidance from seniors and fellow residents.

I’m returning to academics after a gap of around 4 years due to marriage and having a child. During this time, I haven’t had much interaction with my peers or been involved in a hospital setting, and I feel like I’ve lost touch with my medical knowledge. Honestly, my confidence has taken a hit, and I’m quite nervous about starting residency. Sometimes, I feel like I’ve forgotten everything I learned during MBBS.

I would really appreciate it if you could help me understand a few things:

  1. What are the usual duties and responsibilities of a first-year MD Pathology resident? What does a typical day look like?
  2. Is there anything I should study before joining? If yes, which topics or books would you recommend?
  3. How should I approach studying during residency? How do you balance routine departmental work with academics?
  4. What basic skills or concepts should I brush up on before starting? Should I revise general pathology, hematology, histopathology, or something else?
  5. What do you wish you had known before starting your first year of Pathology residency?

I know residency will be a learning process, but starting after such a long gap makes me anxious, and I sometimes worry whether I’ll be able to cope.

Any practical advice, study tips, resources, or words of encouragement would mean a lot to me. Thank you! ❤️

AI-generated WHO Blue Book anki deck

Since there don’t seem to be any Anki decks for the WHO Blue Book, I experimented using Claude to generate one. I prompted it to use a lot of images and tried to make something decent.

Here is the thoracic chapter as an example. It is separated into chapters, and you can always feed it back into AI to customize it into a certain style, trim it down, add more detail, etc.

Thoughts or suggestions? I know some people here aren’t fans of Anki or AI, but I think it’s a decent use.

Residency Application Best PSTP Pathology Programs for MD/PhDs?

Hi, I am entering MS3 after finishing my PhD at a Tier 1 school. I want to become a professor and a pathologist - either molecular, transfusion med, or hemepath, and think pathology PSTP is the best option for me. Which programs are the most prestigious PSTPs? I plan on CP only and following the path that several CP only faculty leading basic science labs have done. My background is in cell biology.

Amongst UW, Stanford, UCSF, Hopkins, MGB, Michigan, WUSTL, Yale… which are places that people look at and say “WOW that person went to X for CP PSTP.”

Sorry I know this is a super vain question but I doubt I’m the only one thinking this and would just like current attendings, residents, fellows, etc to give some clarity.

There’s also a wide range of wowness in terms of folks I’m seeing on various websites for their PSTPs. Like some have first author CNS and like kinda well known in their field already… others barely have first author papers but are in the same program. How does that happen?

Job / career FRCPath chemical pathology— Need guidance on the UK Clinical Biochemistry/Chemical Pathology pathway and FRCPath preparation

I am planning to pursue MD Biochemistry in India, appear for FRCpath and enter work in the UK.
I would really appreciate guidance from anyone who has pursued this route or has any idea about how this pathway works.

My main questions are:

Career pathway: After completing MD Biochemistry in India, what are the possible routes to working in Clinical Biochemistry/Chemical Pathology in the UK? Would I need additional training or qualifications before I could work in the NHS?
FRCPath eligibility: Can apply for FRCPath Clinical Biochemistry Part 1 while still in postgraduation? Are there any specific eligibility requirements or recommended training experience I should know about?
Study resources: Which textbooks, question banks, online resources or mock tests would you recommend for FRCPath Clinical Biochemistry Part 1?
Coaching and courses: Are there any genuine online coaching programmes, recorded lectures, revision courses or mentoring opportunities specifically for Clinical Biochemistry/Chemical Pathology PART 1 and PART 2? I’ve come across several FRCPath courses, but many seem to focus on Histopathology rather than Clinical Biochemistry.
Career prospects: How realistic is it for an IMG with an Indian MD Biochemistry to enter this specialty in the UK? What additional steps would make the transition more feasible?

I’m hoping to start preparing during my MD or soon after completing it, so any advice about when to begin would also be very helpful.

I would especially appreciate responses from IMGs who have taken FRCPath Clinical Biochemistry, doctors currently working in UK Clinical Biochemistry/Chemical Pathology, or trainees familiar with the recruitment process.

3 mm nodule in otherwise unremarkable thyroid lobectomy plus bonus question

Thumbnail gallery
19 Upvotes

Curious about your opinion on this case. Patient is a young woman, clinical diagnosis was parathyroid adenoma, which was not found during surgery, so a lobectomy was performed and additional fatty tissue was removed. No prior thyroid history, no prior FNAs. There was a whitish nodule in the thyroid about 3 mm in diameter.

Sorry about the thickness of the section, I hope you can appreciate the at least PTC-like nuclei and the "possible capsular invasion" area?
How would you sign this out? WD-UMP? NIFTP? Adenoma? Reactive?

Bonus question: intraoperative PTH fell rapidly, but no parathyroid tissue was found in the wholly submitted thyoird tissue or fatty tissue (which had some lymph nodes). I read this can rarely happen but want to make sure I'm not missing something.

Clinical Pathology Would you try AI clinical interpretation for lab analyzer reports?

Hey everyone! I’m exploring an idea around using AI to generate clinical interpretations from lab analyzer reports (hematology, biochemistry, immunoassay, etc.) and classify cases by risk level.

If there were a tool where you could upload a de-identified report and try the AI interpretation yourself, would you be interested in giving it a shot?

Would love to hear your thoughts, especially what would make this useful (or not useful) in your daily workflow!

Job / career Are GI Pathologists in demand?

Hello! Haven't started medschool yet or even applied but I'm thinking of what I want to do in the future. Pathology has always been an interest of mine and I'm thinking of GI pathology. Is it in demand? I've been sort of researching and I don't see any job listings or much information about them. How's the pay and hours? How would I apply to a private institute?

Can you pursue pathology with zero interest in research or wet lab work?

Current MS3 strongly considering pathology. I’ve noticed that many residency programs heavily emphasize research on their websites and seem to have a lot of MD-PhDs.

Is research typically required during residency? Can you realistically pursue pathology with zero interest in research and a goal of working in private practice or hospital settings ?

histo tech to USA medical school

Hi! I’m really interested in becoming a histology technician. I currently live in South Texas and am considering working as a histology tech during my gap years while preparing for medical school. For anyone who has gone through the schooling and training, do you feel that it was worth the investment? I’d also love to hear from anyone who worked as a histology tech before transitioning to medical school. What was your experience like, and would you recommend this path? Thank you! Also, I have over 2 years of undergrad research (IHC, tissue processing) :)

Path assist jobs

My son is a forensic science major and really interested in working doing autopsies. If he decides to go to Pathology Assist school would he be able to work in forensic pathology with this education or do they really work moreso in a medical lab?
