NEWBIE ORIENTATION: CPPS vs Prostatitis

The vast majority of prostatitis cases are non-bacterial, i.e. NIH Type III non-bacterial prostatitis. Expert consensus (of the research) estimates this number to be around ~95% of all cases. True chronic bacterial prostatitis (CPB) is rare. Read more about the prevalence of CBP here, complete with journal citations.

CBP also prevents with unique and specific symptoms. Here is how to identify bacterial prostatitis based on symptoms.

Q: If I don't have an infection, then why do antibiotics make me feel better? FIND OUT WHY

The rest of us have (or have had) NIH Type III non-bacterial prostatitis, now referred to as CPPS or UCPPS - (Urologic) Chronic Pelvic Pain Syndrome. Type III non-bacterial prostatitis can present either with or without actual inflammation of the prostate, but overt prostate inflammation is very uncommon. Most men with CPPS (non-bacterial prostatitis) have small, firm, 'normal' prostates upon examination. This means that the common 'prostatitis' diagnosis is very often a total 'misnomer,' as most cases have no prostate inflammation whatsoever.

While CPPS is officially a syndrome (The 'S' in CPPS), or a collection/pattern of symptoms with no singular cause agreed upon by the larger medical community, there are top theories with high quality evidence behind them. And importantly, most syndromes nowadays are being categorized as variations of central sensitization (ie nociplastic mechanisms) - including IBS, CFS, POIS, RSS, etc.

The top theory backed by research: CPPS is a psycho-neuromuscular chronic pain + dysfunction condition. It often affects the muscles of the pelvic floor, the peripheral nerves that innervate the pelvic region, and the central nervous system (which includes the brain and spinal cord) - among others. This means that treatment requires a multi-modal, integrated treatment approach, and that there is no single pathway or 'pill' to recovery.

I must emphasize that the central nervous system (ie centralized/nociplastic mechanisms) of CPPS affect at least 49% of all cases according to the MAPP study (Multidisciplinary Approach to Pelvic Pain). Do not neglect these. We recommend reading the centralization section below 👇

RECOMMENDED: 1. Centralized Pain Criteria and Citations

  1. Psycho neuromuscular CPPS - with journal citations and techniques to apply.

Things that are known to trigger CPPS (chronic pelvic pain and dysfunction)

These commonly happen via central (nervous system) or peripheral (pelvic floor or nociceptive/neuropathic) mechanisms

  1. Pelvic injuries (falls, hernia, accidents)
  2. Perceived injuries
  3. Infections (UTI/STD)
  4. Stressful experiences and trauma, including sexual abuse/assault
  5. Regretful/anxious sexual encounters
  6. Poor bowel/urinary habits (straining on the toilet often - constipation) or holding in pee/poo for extended periods (like avoiding using a public toilet)
  7. Poor sexual habits (edging/gooning excessively)
  8. Cycling or intense gym habits

SYMPTOM VARIABILITY:

CPPS also presents differently from person to person, and you may exhibit only a few symptoms from the total 'pool' of possibilities. For example, you may only have a 'golfball sensation' and some minor urinary urgency. Another person may have tip of penis pain, testicular pain, and trouble having bowel movements. A third may have ALL of those, and also have sexual dysfunction (ED/PE) and pain with ejaculation. But they are all considered to be CPPS. Here is the full list of symptoms of non-bacterial prostatitis (ie CPPS) - https://emedicine.medscape.com/article/456165-clinical?form=fpf

The chief symptom reported by patients with abacterial prostatitis/CPPS is pain. Genitourinary symptoms include perineal, penile tip, testicular, rectal, lower abdominal, or back pain.

Patients can also have irritative or obstructive urologic symptoms such as frequency, urgency, dysuria, decreased force of the urinary stream, nocturia, and incontinence. Other symptoms are a clear urethral discharge, ejaculatory pain, hematospermia, and sexual dysfunction.

Note: If your symptoms extend BEYOND the pelvis, this is a classic indication of centralized mechanisms (ie nociplastic mechanisms) - What some doctors have in the past called "central sensitization." According to the American Urological Association, these include symptoms like headaches and migraines, IBS, fatigue, fibromyalgia, and more.

So how do we treat it?

The most evidence based approach to treatment is called "UPOINT," a treatment/phenotyping system for Prostatitis/CPPS that was developed by the American Urological Association. UPOINT Stands for:

Urinary, Psychosocial, Organ Specific, Infection, Neurologic/Systemic, Tenderness (ie, Muscles)

it's been shown to be very effective (around 75%) in treating CPPS, as it takes each patient and groups them into phenotypes based on symptoms, then treats them in a customized, integrated, and multi-modal manner. Every case is treated uniquely by symptoms, and this leads to much better patient outcomes. UPOINT is what a good urologist uses to treat patients with CP/CPPS. If your urologist isn't aware of UPOINT, find a new one. You're probably not in good hands. Citation: https://pubmed.ncbi.nlm.nih.gov/34552790/

✓✓✓ NEW SUFFERER TREATMENT CHECKLIST

ENGAGE WITH A PHYSICIAN:

  • Do see a urologist to rule out any serious structural issues
  • Do get a LUTS and/or bladder ultrasound (check residual urine/voiding issues) along with a DRE for prostate size assessment
  • Do get a urinary culture and/or EPS localization culture, if infection is suspected (based on symptoms) - AUA guidelines DO NOT recommended semen cultures - full text, page 21
  • Do get any physician-specified blood tests
  • NOTE: Cystoscopy is typically reserved for suspicion of IC/BPS - but not typically recommend for CPPS
  • Do not use antibiotics without meeting specific diagnostic criteria. Only ~5% of all prostatitis cases are bacterial (even less if your case is > 90 days)

! ! WARNINGS ON INDISCRIMINATE USE OF FLOROQUINOLONE ANTIBIOTICS (Like Cipro or Levo) ! ! Click to Read FDA & EMA Warnings

Thinking about MicrogenDX testing? Please think again, the 2025 AUA Guidelines specifically advise against it's use: READ OUR MOD MEMO

ENGAGE WITH A PELVIC FLOOR PT - Muscles and Nerves

  • See a pelvic floor physical therapist, one who has experience TREATING MEN and can do INTERNAL AND EXTERNAL trigger point release. Studies suggest that 47% - 90% of CPPS cases have pelvic floor myalgia (pain, tenderness, trigger points), and multiple studies show 70-83% of people improve significantly with pelvic floor physical therapy
  • Practice diaphragmatic belly breathing daily
  • Practice pelvic stretching daily (and combine with the breathing)
  • NOTE: 2025 AUA Guidelines suggest that ESWT, acupuncture, dry needling, and TENS help some cases

CENTRALIZATION/BIOPSYCHOSOCIAL:

  • At least 49% of cases have centralized/neuroplastic mechanisms according to the MAPP research network study
  • EXTERNAL: Manage and reduce stress and anxiety in your external environment (work, relationships, finances, etc.)
  • INTERNAL: Address the fear towards your own symptoms. And, avoid obsessive preoccupation & problem solving with symptoms, redirecting your attention to things that are meaningful and enjoyable (distractions and hobbies)
  • Belief/perception of safety or danger (including assumptions about assumed injuries or assumed infections) is also shown in studies on chronic pain to affect our physical pain experience
  • Take time for yourself and do things to relax and engage in self care. Find SAFETY in your body again: mindfulness/meditation, yoga, baths, etc
  • See a chronic pain therapist, coach or psychologist who practices PRT, EAET, and/or CBT: Examples: Pain Psychology Center (LA), the app "Curable" for chronic pain/symptoms (Note on CBT - this is typically found less helpful for pain in controlled experiments, compared to newer PRT and EAET)
  • Recommended readings: Alan Gordon (LCSW) - 'The Way Out' or Dr. Howard Schubiner 'Unlearn Your Pain'

Urological (Pharmacological) Treatments to Discuss With A Doctor:

  • Discuss alpha blockers (Alfuzosin etc) for urinary/flow/frequency with physician, if you have urinary symptoms. Be aware of possible side effects in some users: PE, Retrograde ejaculation, etc
  • Alternate to above, if they don't work for you or you have side effects, discuss Cialis with your physician. Cialis (Generic: Tadalafil) also helps with ED and can be used at low doses of 2.5mg/day.
  • Discuss low dose amitriptyline (off label usage) with your doctor, which can help approx. 2/3 people to relieve the neuropathic pain associated with this condition
  • Discuss rectal suppositories for pain management, often containing meds like: diazepam (Valium), available via a compounding pharmacy - this is a controlled substance; always discuss with your doctor - not meant to be used daily.
  • You may try NSAIDs for pain during flair ups, but caution for daily, ongoing use. MOST find this class of meds unhelpful.
  • Oral Steroids are NOT RECOMMENDED, per 2025 AUA Guidelines

HERBS/SUPPLEMENTS:

  • Phytotherapy (Quercetin & Rye Pollen, ie Graminex) - highest level of evidence for CP/CPPS
  • Magnesium (glycinate or complex) - less evidence
  • Palmitoylethanolamide (PEA) - less evidence

BEHAVIORAL CHANGES (Lifestyle): Please note that these suggestions cast an extremely wide net, and many do not apply if symptoms are centralized/nociplastic.

  • Avoid edging or aggressive masturbation; limit masturbation to 2-3/week, and be gentle. No "Death grips"
  • Less sedentary lifestyle - walk for 1 hour daily or every other day (I would recommend you build up to this, start with 15 minutes daily, easier to start a habit with a gentle, but regular introduction)
  • Get your blood pressure, body weight, and blood sugar under control (if applicable)
  • Gym goers and body builders: lay off the heavy weights, squats, and excessive core workouts temporarily. Ask a physical therapist to 'OK' your gym and exercise routine. This is a possible physical trigger
  • Cyclists and bikers: Lay off cycling until your physical therapist OKs it - this is a known physical trigger
  • STAND MORE! Get either A) a knee chair, or B) an adjustable standing desk. You'll still need the regular chair, because you can't sit on a knee chair or stand all day, basically, although conceivably you could do both A and B, and skip the regular chair
  • Try a donut pillow if experiencing pain while sitting

BEHAVIORAL CHANGES (Diet) Note: Dietary triggers affect a small MINORITY of cases

  • Try reducing/eliminating alcohol (especially in the evening, if you have nocturia)
  • Try reducing/eliminating caffeine
  • Try eliminating spicy/high acid foods
  • Try eliminating gluten and/or dairy
  • Try the IC Diet (basically this is all of the above, and more)
  • If eliminating or reducing doesn't help, then it probably doesn't apply to your case, enjoy your food and drinks!

Tony's Advice for Beginners

Top Rated Thread of all time in this Reddit: The experience of an MD with CP/CPPS

Antibiotics

Every day numerous questions are posted here about the effects of antibiotics. How can my case be nonbacterial if antibiotics help me (for a while anyway)?

The simple fact is that antibiotics are ANTI-INFLAMMATORIES and also have other immunomodulatory effects. In fact they are used for these effects in many conditions (acne and other skin conditions, ulcerative colitis, Crohn's Disease, and more).

Sadly, even many doctors don't know this (it was only acknowledged this century and medical school curricula have mostly not been updated yet). But the research is all there. (Note that due to our genetic differences, some people react more to the anti-inflammatory effects and some people less, or not at all. This is known as pharmacogenetics).

Acute bacterial prostatitis does happen, and it's pretty obvious: very sudden abrupt onset, fever, chills, nausea, vomiting, and malaise (feels like having the flu). Nothing like what 99.9% of readers here have. It's often a medical emergency that requires a trip to the ER.

But you may still think your case is bacterial, perhaps a chronic and not acute case. Professor Weidner says:

"In studies of 656 men with pelvic pain suggestive of chronic prostatitis, we seldom found chronic bacterial prostatitis. It is truly a rare disease."Dr. Weidner (Professor of Medicine, Department of Urology, University of Giessen, Giessen, Germany)

Chronic bacterial prostatitis also has a distinct picture. It presents as intermittent UTIs where the bug is always the same (often E coli). Here's an example:

I have chronic bacterial prostatitis that responds well to antibiotics. ... The doctor will express some prostate fluid and run a culture to determine the bug and prescribe an appropriate antibiotic. My bug has consistently been shown to be E-coli.

That being said, my symptoms usually start with increased frequency of urination, burning and pain on urination, and pus discharge. But no pain other than that and it usually goes away after a few days on the antibiotics. I continue the antibiotics for 30 days which is well after the symptoms have disappeared. I can usually expect a relapse in 6 to 12 months. ... This has been going on for more than 30 years. .... My worst experience a number of years ago was when I thought I would tough it out and see what happened. The pain got excruciating, testicles inflamed, bloody discharge, high fever. But this responded well to antibiotics and I haven't tried to tough it out again after that experience. I know when it starts and go on antibiotics right away.

I know that guys who have chronic pelvic pain syndrome may scoff at what I say and I know that they are in the majority. I really don't know what they are going through but then, they don't know my experience either.

So here are the key points to look for in chronic infection:

  1. Relapsing UTI picture (dysuria [painful urination], discharge)
  2. Consistently identifiable bug (the bug does not change)
  3. Generally no pain unless accompanied by fever and discharge. So for most of the time, men with chronic bacterial prostatitis do not have any pain.

All the rest have, sigh, UCPPS (CPPS).

Vent/Discouraged I am in pain and mental breakdown

My story is a little bit complicated, so thanks to everyone reading and trying to understand my case...

Male, 29 years old, married foe 3 years, no kids, due to a mild varicocele in left testicle (very mild, like it's not even a problem for the doctors) 2 years passed, i am 1m80, i used to eat a lot of everything, so unhealthy food and no activity, i work in an office, sitting all day and work all night at home sitting also facing computer, max 5 hours or 4 of sleep, sex was not that great max 3 times a week or 2, i don't last much, in a personal view it was okey to be overweight, although i had some pain in the testicle and my colon, then 5 months ago the tragedy happened, i had genital warts in my penis, i dont know from where, my wife did a frotti she was clean no high risk and not a single wart in her (i envisage for her to do anal scanning in the futur) had my warts eliminated by electrocution, since then my life went downhill with paranoia and i check my penis every second, its okey no sex for 4 or 3 months, only edging and masturbating, then 2 months ago i ejaculated, i felt my penis got enflammed the day after, like a little bit bulky, with no pain, then one day i felt some burning, didn't pay attention to it, and then when i ejaculated again i felt a horrible pain in my meatus and my urethra, like electricity and frequent urination for all night, i didn't sleep or anything, i saw a doctor, he checked me, nothing bad, although i had a constant feeling of burning in my meatus, i did all std tests, urine test, meatus swab for chlamydia and gonhorea etc..., spermoculture, all came up negative, did echography, prostat not enlarged, he gave me anti inflammatory and anibiotic and some cream, felt nice for 5 days, then symptoms returns everytime i masturbate (bear in mind that i always edge ans touch myself) consulted another urologist, he told me its prostatitis, gave me doxyciline, nothing changed, i saw another urologist, redid all the tests and examination, he told me there is nothing, he gave me Ceprolon, last week, saw another urologist, he told me stop thinking about it and dont take ciprolon its just an irritation or mild inflammation, i ignored and still take ciprolon with another anti inflammatory, i suspect a balanitis, because of small decoloration and small bump in the head, saw another urologist, he checked me again (i forgot to mention that i told every other urologist if i have balanitis, they said no) this time i told the urologist about all my concerns and what i did and everything, and that i still feel something in my meatus, i told him mybe i have intra-meatal wart, although my pee stream is normal, a little bit week but no split, he checked my meatus and told me we dont have to use the camera because you have nothing bad and no symptoms, i told him what if i have cpps, he told me no, you dont have pain when you sit, although i feel a mild burning in my asshole sometimes, and told me this "with all my respect go see a psychologist"

Today i feel my penis weak, i cant get really aroused, when i press my the head of my penis and the meatus i feel a little mild sting inside, erection is very hard to maintain, constant burning, i am in a big paranoia, i suspect i have all the disease of the world (two months ago i did keggel exercice so harshly i wanted to improve my lasting time) to the point i suspect mybe i ruptured the muscle of my meatus but when am erect nothing hurts, am afraid to ejaculate, sometimes i test myself that mybe am good, but no, i feel something there (i stopped sugar and unhealthy food 2 months ago and i hit the gym 3 times a week) i wanted to improve, yes my varicocele is not hurting and my colon also, i feel great, but my penis no, 5 urologist said am good there is nothing there, but i can't agree, i have this burning, this urge to see what's inside, am taking all the medicines that exist, i cut off coffee and excessive smoking, i drink only water, what can i do? Where is the problem? I cant feel relief, is that a classic prostatitis? Does the prostatitis makes the meatus burn? Can a prostatitis be without enlarged prostat? Should i wait and not touch anything for like 15 days and do the camera? Do i have stenosis? Am in so much pain and big spiral.

Am very sad and concerned.

“Anyone else stopped waking up with morning wood despite having normal erections?”

Hey! I’ve posted here before. I have a lot of strange symptoms that I think are largely due to central sensitization. For example, I have chest pain, ankle pain, and a clicking sound when I swallow.

I also have pelvic issues. Typical CPPS symptoms.

Weak urine stream, pain after ejaculation, increased urinary urgency, etc.

But I noticed something after reading a random Reddit thread.

I almost never wake up with morning wood anymore. I actually sleep really well, through the entire night without waking up.

I sleep from 10 PM to 6:30 AM without waking up, but as soon as I wake up, the first thing I have to do is go to the bathroom.

It suddenly hit me that I don’t get morning wood anymore.

Other than that, I have no problems with erections.

Anyone else experience this?

Vent/Discouraged Mild pain near my left testicle that comes and goes. Has anyone experienced this?

Hey guys, need some advice.
I’m 22M, and yesterday night I noticed a mild, unusual pain around my left testicle and the left side of my penis/pelvic area. It’s not severe, but I can feel it when I touch the area.

The strange thing is that I experienced something similar about a week ago. It went away on its own by the next day, and the same thing happened this time. This morning, I don’t feel any pain at all.
I searched online, and I came across things like testicular torsion, which honestly made me a bit worried. I haven’t experienced vomiting or any other obvious symptoms, just this mild pain that comes and goes.
I’m a bit hesitant and embarrassed to bring this up with my family or consult a doctor, especially since the pain has disappeared. I wanted to ask if anyone else has experienced something similar and what the cause turned out to be.
For some additional context, my penis naturally tilts slightly to the right. My left testicle also sits a little higher than my right one.
Has anyone experienced recurring mild pain like this?
Any advice would be appreciated.

Thanks in advance.

Vent/Discouraged Help, confused?! What can I do?

Hello,

Me and my wife are experiencing difficulty TTC. My semen analyses keep showing bacteria and excess cells. I had a sensitive semen culture that found E Faecalis that is thought to be of the prostate/male accessory gland.

I have no symptoms, no pain, other than infertility. My understanding is that infection of the male accessory glands can affect semen quality.

I took 4 weeks of antibiotics, twice per day, and had a repeat semen culture, and the infection is still present?

I appreciate I’m not in pain but we believe this is be affecting my semen quality. What do I do?

Before I leave Reddit (I’ve become addicted)

I’d like to ask for anyone’s best tips for relieving pain, and eventually curing pain.

This has gone on way too long and I feel incredibly hopeless.

Anything you did heal these conditions

I want no negativity.

Possible prostatitis

Hello people,

Male 37 here

1.5 month ago I had an episode of bacterial infection. The symptoms were fever for two days, fatigue and frequent urination. I took doxycycline (due to an episode of unprotected sex I had) for ten days and since then I started feeling better day by day. In the meantime the ultrasound showed enlarged prostate 35.6 ml. The only symptoms persisting were frequent urination and nocturia. After a month though and while everything was going better the pain returned in my genitals area and the frequent urination got worse.

Am I dealing with chronic prostatitis? I'm waiting for my culture results and I have a cystoscopy scheduled.

Have you dealed with something similar?

Thanks for the attention in advance.

PBNO Problem. Need advice on whether to get surgery or not

I am diagnosed with PBNO problem in Dec 2024. Doctor conducted urodynamic study and explained me that I was diagnosed with PBNO and needs a minimal surgery of BNI - Bladder neck incision surgery.

For now doctor told to use Alfoo 50 mg, Megatas 50 mg and gave me Urimax T medicines and in Jan 2026 doctor changed the medicines to Alfoo 50 mg, Mirago 50 mg said no need to use Urimax T if I am not getting ED.

I am getting erection but right after getting erected it starts going back to normal in front of my eyes and only if I give it a push it again erects. Right after ejaculation in just seconds it goes back to flaccid nature.

I started getting pain near liver, right abdomen. I keep getting urination feeling for every 1.5 to 2 hours. I am getting good sleep but I generally wake up between 3 to 5 AM to urinate. If I fail to wake up I will get burning sensation in abdomen and it is a big discomfort.

I am married and my wife recently had a miscarriage and Doctor advised me to take Ejaculation sparing BNI. I am thinking to go to surgery after having my first child but this problem is causing a great deal discomfort. I am not sure on how to proceed here. If I take BNI, its highly difficult to have kids. If I get ejaculation sparing BNI surgery I am not sure if that works out or it is a risk that I have to get full surgery in future.

Can anyone help or guide me in proceeding to surgery is better or not.

Vent/Discouraged How to treat prostatitis at home

M32. Semi-active. Not overweight. Moderately healthy. Health conscious.

There’s a slight percentage that I could have prostatitis, but need to do a Post Residual test first to check bladder’s emptying ability. Doc referred me to urologist as well to get better expertise on that area.

I’ve done STI, Trich, Culture Sensitivity, Uirinalysis, etc. all normal other than indication of inflammation somewhere. At this point, it’s annoying to not know what’s causing it…

My question is, if it is, can I treat it at home? What’s your experience?

Positive Progress A word of encouragement

Wow. What a hectic year. Since around April February 2025 my world was turned upside down by this condition, but also the catastrophic back injury I suffered 6 months prior. Baffling to doctors, my prostate slowly became dysfunctional after the back injury (which scared me that I may have Cauda Equina). By Feb last year I was basically just dribbling/drops to pee and it felt like someone was constantly lifting me up by my groin- pulling me up by my insides. After multiple visits to the Emergency over the period of a few weeks, I was pushed to the brink. The pain, pulling, no sleep and mental toll was too much and on a Saturday in March I was in a very serious place until my partner and friends intervened and told me to go back to Emergency and not to leave until more was done. That it was unacceptable to just keep checking for Cauda Equina and UTI's without helping much more than pain relief. So I did, and was given a course of steroids, plus other anti-inflammatories, plus a litany of other things. It took the edge off but not much more.

Well consistently now for a few months my urine flow has returned a lot more. Sometimes I'm close to having the flow I had before the injury while other times I still feel a lot of pressure just need to be patient at the toilet and wait for it to allow a flow. But way better than before. Erections are still painful at times and I may never have one that's more than 80% but my partner is patient and kind and she loves me, so that helps with anxiety over sex. The pulling feeling still exists but a lot more manageable for the mind to navigate. I take Prostate Suppliments and drink water regularly but have trained my brain to not overly exert to try to pee. So every morning I pee and go on live life until I feel the need to pee again and relax into that. It took months and months to not always rush to the toilet to empty out drop by drop. I do regularly reverse kegels and massage the prostate area. I did get a cystoscopy done and they did find a high bladder neck but no stricture, which was baffling as I'd had incredible urine flow before the injury. My theory is that the back injury changed the musculature in that area but I'm no doctor or urologist. Just a very alarming mystery. Curiously enough, haemorrhoids made the situation infinitely worse, which is actually why I'm posting now, as I've had a haemorrhoid flare now but mentally am in such a better place that I'm riding through the discomfort much better.

And so I want to thank everyone on here who have posted encouraging posts, the mods who are always engaging and helping..and I also want to encourage anyone who is going through this or who have family members going through this, that with time, patience and dedication to improving, even if it's slow and tiny step progress, you will get to a much better place. Also with the right professional help (here in Australia I found a wonderful guy who helped with relaxing that whole downstairs area of my body) you can navigate the whole muscular side of this condition, which for me obviously plays a massive part.

Take care

Vent/Discouraged Is it back or is it an actual infection

Hi all back in 2024 I was diagnosed with prostatitis.

Last year I had to go get scans for my kidney as they thought I had kidney stones with the discomfort I was in

I had been given antibiotics, and it clears up the discomfort.

So last couple of days , lower back pain, sore left flank when pressed, and slightly tender bladder area when pressed - that's it

No pain when I pee, no burning, everything else is fine. No fevers or anything else.

Was at the gym today did my work out no bother, and I'm walking 10k plus steps every day Was out and did a 15mile mounting biking session yesterday

I feel perfectly normal apart from this discomfort

Is this a sign of prostatitis being back. Id rather not go on antibiotics if I really don't need too

Vent/Discouraged Complications post Urolift procedure

I feel like the last week has been a nightmare that I keep expecting to wake up from.

I'm 37 years old and was diagnosed with BPH last year. It was impacting my urination to the point where I had mild hesitancy and very prolonged dribbling at the end. Occasionally this would result in wet pants.

My doctor suggested urolift. I read through studies online and it sounded good. Exactly like what I needed. I wish I had cancelled and just lived with urination issues for the rest of my life.

On Wednesday, I had the procedure and was told it was a success. I was able to urinate so they didn't give me a catheter. Sent me home without antibiotics or any further medications.

Every single time I went to the bathroom it felt like I was pissing daggers. I was screaming. 10/10 pain. That was day one. Day two, I begged for pyridium and my doctor prescribed it. It did not help.

The next day, I could no longer urinate. I was getting droplets. I called my doctor's office and they had me come in for a catheter an hour later. They took a urine sample but I won't know if the pain was from an infection until Monday because the labs are closed on the weekend.

The entire day I was unable to get comfortable. Couldn't walk without tugging on the head of my penis. Had horrible bladder spasms. They prescribed my Norco and a medication for the spasms.

The following day was even worse. I had my first bowl movement and had a decent amount of blood leave my urethra outside the catheter line. My penis was covered in blood. I called and they said: 'that isn't typical but it is normal'

What in the hell does that mean?!

I just need someone to tell me if this is something they experienced and if it really does get better. I'm going crazy. I can't even sit because of the pressure from my prostate. Everything is infinitely worse than it was pre op. I can't find a comfortable position in which to sleep. I can't walk about. I'm fucking stuck in bed and the Norco is only doing so much for the pain.

I feel like I was tricked into some bullshit surgery.

Trouble urinating HELP 😢

Hi guys new to this sub and really need everyone’s help! I’m a 20yo male that has been having extreme trouble urinating for the last 2 months after taking a 20mg dose of Amitriptyline and it has honestly ruined my life I can’t use the bathroom!! And when I do empty my bladder I have this Constant sensation and uncomfortablity in my penis . When I went into hospital they said my urethra looked inflamed but that’s about it. Could this be prostiritus? Has anyone here experienced this and how long has it taken for them to recover and will I ever recover ? 😭

Watery semen and no orgasm pleasure after UTI or prostatitis symptoms. Has anyone recovered?

In late September, I suddenly developed high fever, chills, severe lower back pain and burning urination. My urine test showed elevated white blood cells, but the culture came back negative.

I was prescribed nitrofurantoin for 7 days, followed by trimethoprim for another 7 days when my urinary symptoms returned.

Thankfully, my fever and urinary problems have now resolved. My urine flow is strong, and I still get normal erections.

However, my ejaculation has changed significantly.

My semen is much thinner and watery compared with before. Ejaculation is still forceful, but the stream is extremely narrow, almost like a syringe.

Most concerning, I can ejaculate but feel absolutely no pleasurable orgasm sensation.

These changes started around the illness and have continued despite improvement in my other symptoms.

Has anyone experienced watery semen, a narrow ejaculation stream or ejaculation without orgasm pleasure after a UTI or prostatitis?

Did you recover fully? How long did it take, and what actually helped?

My wife and I are hoping to start a family, so this has been really upsetting and making me depressed.

We have a gp appointment booked for family planning but in the meantime I would especially appreciate hearing from anyone who experienced similar symptoms and recovered. Please help me thank you 🙏

CPPS from edging: tight perineum & urine dribble. Did abstinence cure you?

Hi everyone,

I’m posting here because I’m getting really anxious that I might have developed CPPS, and I’m looking for some advice or similar experiences.

For the past few months, I’ve been dealing with two main symptoms:

  • A constant feeling of tightness/contracture in my perineum.
  • Post-void dribble (some drops of urine always stay trapped and leak out right after I finish peeing).

I am almost certain this condition started after a period of doing frequent and prolonged edging sessions. This has been going on for months now, and I suspect it hasn't gone away because I never truly stopped masturbating (even though I am much gentler now and my sessions are shorter).

My questions for this community are:

  1. Has anyone else developed these exact symptoms (pelvic floor tightness and trapped urine) specifically from edging?
  2. If you were in my situation, did completely abstaining from masturbation give your pelvic floor the time to heal and resolve the issue?

Any advice or success stories would be really appreciated. Thank you!

Sharp Cramp/Pain after abstinence in masturbation

Hi, I have noticed a weird recurring pattern that has happened thrice now for the last 3 months. I usually masturbate regularly but whenever there's a big gap in between, then on the day i masturbate again, once I ejaculate, after a few seconds, there's a sharp cramp/pain in urethra which lasts for few seconds and than disappears. If I masturbate regularly, it doesn't happen, after a gap of 2-3 days also, it doesn't happen, for a long gap like 5+ days, it happens. Anyone knows what's cause of this and how do you fix this?

How to test for bacterial

Hello, I am fairly confident that I have bacterial prostatitis, but am unsure how to test for it. My symptoms include persistent dehydration and foamy, smelly urine. These urinary symptoms have been coming and going every few months since I had mycoplasma genitallium about a year and a half ago. I have experienced CPPS symptoms as well and understand it is common to be paranoid, but am pretty confident something else is wrong or else my urine would not smell and I would not be so dehydrated all the time.

I saw a urologist earlier this year because I was also experiencing hematospermia (blood in my semen) during sex. He assured me it was highly unlikely I had bacterial prostatitis, and that was likely the mycoplasma still working its way out (nearly a year later). Said that if it was an issue he would’ve been able to tell from my urine culture. Said that doing a semen culture would be pointless would not be able to tell anything.

After a seemingly normal summer I am back to having these urinary UTI symptoms again yet testing negative for a UTI. I am currently on a week of antibiotics for BV but it should not be causing these issues.

Is four-glass or two-glass test what I am looking for? I have been trying to figure this out nearly two years now and just been robbed by doctors and lost in loops of searching for a solution on Reddit. Can somebody please tell me how to test for this so I can get piece of mind (I’m in the US)

Weight lifting after CPPS

Has anyone been able to lift weights again after being diagnosed?

For me personally, lifting something off the ground or "farmers carrying" even my lunch pale from work will cause immediate urge to urinate which is obviously difficult and light stream or causes a full blown flair up.

Ive always been skinny so lifting weights is a confidence thing for me and ive lost allllllll my gains.

Im contemplating just lifting again, slowly obviously and lightly, and if I piss a bit then so be it. I dont know. Anyone with advice?

has anyone tried injections for chronic pelvic pain?

hi, first of all, i want to clarify that i'm not the one dealing with chronic pelvic pain, my boyfriend is. he's been struggling with it for about 10 years now. over the years, he's seen countless doctors, tried different medications, and even undergone surgeries, but somehow the pain always finds its way back. recently, a new doctor suggested trying injections/nerve blocks, and i was wondering if anyone here has gone through something similar.
if you've tried them, i'd really appreciate hearing about your experience. did they help? how long did the relief last? was the procedure painful? did you experience any side effects? honestly, any information would be helpful.

but there's something else i'd love to ask, especially to those of you who are dealing with chronic pelvic pain yourselves. what's the best way for a partner to support you without making you feel pressured, overwhelmed, or like you're constantly being reminded of your condition? before we even officially started dating, i spent a lot of time reading and learning about his condition, how it affects his daily life, and what being in a relationship with someone who experiences chronic pain might look like. i wanted to make sure i understood what he was going through as much as someone who doesn't experience it personally possibly could.
i love him, and supporting him is something i genuinely want to do. but i'm also aware that no matter how much i read or research, i'll never fully understand what it feels like to live with that pain every single day. and of course, i've talked about all of this with him first. i know everyone's different, and he's already told me what he personally needs from me. i'd just love to hear other people's experiences and perspectives too, since i think there's always something new to learn. sometimes i wonder if i'm doing enough, or if there are things i could be doing differently that would make his life a little easier. i don't want him to feel like he has to be okay for my sake, and i definitely don't want my attempts to help to become another source of pressure for him.
i guess my question is, if you're dealing with this condition, what do you wish your partner understood better? what kind of support actually makes a difference? and are there things people do with good intentions that end up being more frustrating than helpful?

i'd really appreciate any advice, personal experiences, or even little things you wish someone had done for you.

thank you for reading, and i genuinely hope everyone here finds some relief :)

My ongoing nightmare for over 7 years. Kindly read & advise please 🙏🏼

I had a one-night stand with someone. I remember while receiving a blow job the lady cleared her throat & spat on my penis head (I would like to note I did not recall this in the upcoming weeks after this happened)

The next few weeks I had penis pains. I kept masterbating as my retarded doctor told me not to worry & it’s in my head …

This is where everything got worse. My penis turtled, my balls retracted & I got hard flaccid. I also cannot feel orgasms.

Over the months and years my brain remembered she cleared her throat and spat on my penis and this started this nightmare.

I then somehow got a grade 3 varicocele which I had surgery to repair.

I now am doing a lot better mentally. I’d say it took about 6 years to recover mentally. However I now suffer with hard flaccid, ED, no orgasm feeling & occasional penis pains.

What is the best steps I take to diagnosis this and recover?

Thank you everyone 🙏🏼

Tadalafil/Cialis and CPPS

I visited the urologist yesterday to discuss how to proceed as i had CBP before summer and the bacteria is now cleared after doing urine culture three times.

Now I have pain and burning after urinating that is still left and the muscles in the pelvic region feels hard, mostly at the back but van travel to the front which is CPPS.

He told me it's not the prostate that is the problem, it's the large muscle that the pelvic region rest against that develops pain and inflammation in the nerves

He also told many urologists still give antibiotics even if there is no infection present, it's old outdated school he said, in my case I had an infection before caused by catheter, that's now all gone.

So he prescribed Tadalafil or Cialis and said it would help me after taking it for a few weeks.

Those of you who take it, will it also help releive the burning and other unpleasant nerve pain from pelvic region?

Correct Diaphragmatic Breathing?

Are we supposed to breathe into our belly? Or are we trying to breathe and expand our ribcage, including our sides and back? Which "way" is more beneficial for a hypertonic pelvic floor? Just trying to do this the right way, but hard to do when I'm told both are correct.
